Initial Nutrition Assessment - 90 Minutes

Includes: In-depth diet evaluation (you will receive a questionnaire to fill out prior to this session), goal setting, barriers, education, review (and ongoing review) of food journals, recipes, macronutrients and calorie needs, and more.

Follow-Up Session - 60 Minutes

Includes: In-depth education, counseling to address barriers, food journal reviews, and more!

I’m currently in network with Cigna, Aetna, Anthem BCBS, United Health Care, and Medical Mutual. Please fill out the new client form, and I will be in contact to let you know if coverage is provided by your plan. Even if I am not in network, you still may be able to use your HSA for payment, and I can write a superbill for you to be reimbursed.
